Name: Simple, pick a name. Relate it to something to do with your character if you can.
Rank: Summons shouldn't really be E, D rank so we are assuming your summons would be ranked either C, B, A or S. Think about your summon entirely before ranking it, it is an important part of the application.
Appearance: Describe your summons features and physical appearance, things like their colour, any distinguishing markings, perhaps where their forehead protector is located if they are the type of summon that is loyal to its summoners village too. One of the most important things about a summons appearance is its size, please be as specific as possible. Maximum summon sizes are as followed:
C Rank - 10ft
B Rank - 50ft
A Rank - 100ft
S Rank - 200ft
Personality: Another important field, be descriptive. How does your summon act in certain situations, how does it act around other people as opposed to you. How is your relationship with your summon, elaborate on personality traits of your summon, is it snobbish, is it lazy or is his energetic. Go into detail on what sort of things it eats and what it likes and dislikes.
Contract Type: There are a few different types of contracts, one of the main ones will most likely be what we would like to call a Formal Contract. Formal Contracts are contracts with a certain species that you can summon when needed, for example Naruto had a contract with the Toads. Another type of contract would be a Personal Summon, which isn't much of a contract at all, the Baku was a personal summon of Danzo late in the manga, this is probably the easiest example. Finally a Clan Contract, which is exactly would you would assume, a contract based around a clan, so essentially the Aburame have a contract with the Kikai, but that is a different sort of contract. Practically its a species exclusive to that clan.
Species: Pretty simple, if its a Toad, Snake, Bird, Monkey, Lion etc. Be absolutely species specific if possible. Currently all types of summons permitted must be animals and species of todays world as opposed to mythological creatures like Dragons but also things like Dinosaurs.
Speciality: In regards to summons there is a limitation on what specialties they are permitted at certain ranking and how many they are permitted. The specialties limitations are as followed:
C Rank - One Specialty. [Tai, Nin Weapon]
B Rank - One Specialty. [Tai, Nin Weapon]
A Rank - Two Specialties. [Tai, Nin Weapon, Gen]
S Rank - Two Specialties. [Any, excluding Puppetry]
Elemental Affinity: Usually would be one of the main five: Katon, Suiton, Fuuton, Doton, Raiton. If it is an advanced element specify, if it has no element just write N/A. If your summon has an elemental affinity than it must be the same as one of the character's and they can only possess a single element So if your characters elements are Lightning and Wind, your summon can only use either Lightning or Wind. Summons cannot possess advanced elements.
Summons Jutsu: Refer to the Jutsu Section for the Template, please note though; like characters, summons also have a limit to how many jutsu they can start with. Further jutsu can be trained once approved but to begin the jutsu limits are as followed. You may think these seem like small amounts but do note you most likely have more than one in the contract and once again I repeat, these are only starting jutsu.

Name: Try to be creative with the names, like everything else. Regarding your pets it might be nice if you had it referring to a personality trait they have or something like that.
Nickname: It is not uncommon for people to give their dogs nicknames, for example if you had a spotty dog you might occasionally call him Spot, though it might not be what you named him.
Age: When considering the age of your pet, consider their size, if you want a full grown pet, age it correctly, if you want a puppy; once again. Age it correctly.
